NSF - NFD Prevention

It has been closely associated with the use of gadolinium-based contrast agents used in MRIs and MRAs, and so far has only occurred in people with renal insufficiency. Because there is no cure for NSF/NFD, and the condition is very resistant to treatment, there is currently a very heavy emphasis on prevention. Patients with moderate to severe renal insufficiency are at the highest risk for developing NSF/NFD. Anyone with renal insufficiency should avoid gadolinium exposure if possible, but for some people, tests using the contrast agent are necessary. In any event, patients with any level of renal insufficiency should be informed of the risks involved, and have the opportunity to decide for themselves, while taking into consideration the advice of their doctors, whether the benefits outweigh the risks. DoseThe experts in NSF strongly recommend that when tests using contrast agents are absolutely necessary for people with renal insufficiency, the lowest effective dose should be used, especially in those with advanced kidney disease. DialysisGadolinium is primarily removed from the body by the kidneys. Therefore, people whose kidneys are not functioning properly do not eliminate the gadolinium as quickly and effectively as patients with normal functioning kidneys. Patients with normal functioning kidneys eliminate one-half of the injected gadolinium in 90 minutes while those with moderate to severe renal insufficiency, the half life of gadolinium is extended to up to 30 hours or more. This allows the gadolinium to undergo a chemical change and become distributed throughout the organs of the body The gadolinium is allowed to remain in their systems long enough to get into the skin and cause NSF/NFD. Hemodialysis immediately after testing may eliminate some of the gadolinium. Typically, two or three, or more, sessions are needed to remove the toxin, but by doing so quickly after testing, gadolinium may not have the chance to spread throughout the body. Hemodialysis may not be safe for everyone. Many doctors believe that patients who are not already in dialysis should not use the procedure just for gadolinium removal, because initiating dialysis can mean a permanent dependence on the procedure. Each person must be individually evaluated, and the benefits and risks of dialysis must be carefully weighed. Different gadolinium contrast agentsAll gadolinium-based contrast agents are considered to carry a possible risk of causing NSF/NFD, but some are more dangerous than others. So far, of the five gadolinium-based contrast agents approved by the FDA, only three have been linked to NSF/NFD. Most cases have occurred after the use of Omniscan, and some after using Magnevist and OptiMARK. Some experts worry that low-risk patients will be overly cautious and go without the tests that they need. Each case has to be considered individually based on risk factors and the level of need for tests using the contrast agent. From the time I could barely see the board from my vantage point at the edge of the table, I witnessed the subtle strategies used by my older brother and sisters at play. I dreamed of the day I was old enough to playhow I’d dazzle them all with my victory! But that day was long in coming. If theres any game that makes kids feel left out, its Scrabble. Because it requires reading, spelling, and vocabulary skills, many parents think Scrabble isnt for the very young. Think again! Parents these days can easily introduce the game to a child as young as four or five years old. With a little time and effort, they can help children even develop advanced skills that many adults dont know. After the Official Scrabble Players Dictionary, the best Scrabble tool introduced by Hasbro in recent years is the Scrabble Junior Edition. This by far is the best way to introduce Scrabble to your four year old. Play resembles regular Scrabble, getting children accustomed to the flow of the game. First, children draw seven tiles from the pool. In turns they lay down tiles from their racks to match the letters on the board. Letter by letter they eventually spell out entire words, winning points. By games end, they will have used over a hundred tiles to work a variety of different words. Few preschool teachers could match that for reading practice! Children who have mastered this simplified version can then graduate to the next level of play by flipping the game board over to reveal another basic version of the classic Scrabble game. Using this board and modified scoring rules, children progress in difficulty and skill level. By eight years old, most children will be ready for regular Scrabble. Dont get caught up in talking rules and strategy at first. Just jump right in and let your child experience the game for him or herself. Even better, you might play an opponent as a team, allowing the child to suggest plays and explaining tactics as the game goes on. Even if its not the best play, use your childs suggestions occasionally so that he or she feels like a valuable member of the team and experiences first-hand the triumph or failure of a move. Having a teammate to share the disappointment will help when moves arent successful, or when the child doesnt win. Particularly in their early years, allow children to use a dictionary while playing. One rule variation used in my home while growing up was that players were allowed to browse the dictionary for word options as long as it wasnt their turn. This way, young players didnt get bored waiting for their turns, while they acquired a great learning skill! Dictionaries are a terrific safety net and their use can help children broaden their vocabulary base. Outside actual play there are a few games you can teach that will help children sharpen their Scrabble skills. Using the Scrabble tiles, have your child spell out his or her name. Add up the score. Then spell out the other names of friends and family members, cities, states, countries, or other favorite words, adding and comparing the scores of each. Play for fun, taking off the competitive edge, and allow the child to explore the value of different word options. Few skills are more important than anagramming to a Scrabble novice, and this is a skill you may want to explain as your child progresses. Give them a word and a time limit, and challenge them to find five, ten, twenty, or even more words using only the letters given. As they improve, encourage them to find lengthier options, maybe even offering a reward for using all the letters. Ive used this game frequently in the elementary classroom when my class is waiting in line, and I havent yet found a youngster who doesnt like it. Students especially love comparing their lists. If theyve found a word nobody else found (or a word I didnt) it is especially rewarding. You may be surprised how fast your child picks up on more advanced strategies after introducing Scrabble this way. More importantly, he or she will develop an interest in word play, which is infinitely more valuable than alternative interests in TV and video games. When it comes to the typical Italian dishes, pasta has always been one of the most common and popular ones, accepted all over the world. Among the varieties of pasta, stuffed pasta has a long history to go back to, existing even in the ancient days. Ravioli refers to one of the forms and varieties of Italian stuffed pasta, deriving its name from the verb “to wrap.” Stuffing for pastas can include vegetables, meat, cream, fish, and lobsters. The word “ravioli” also indicates to a shape, a number of recipes have been created for various ravioli preparations. Among a number of mouthwatering lobster raviolis, radicchio in the sauce can be simply wonderful in tempering the sweetness of the lobster in the stuffing. For serving four people the ingredients required would include one lobster, weighing around two pounds, olive oil, half a pound of red radicchio, washed, lightly oiled and seared on a grill till cooked but not burnt, dry white wine and salt, pepper and cayenne pepper according to taste. These ingredients can be used for stuffing, while for preparing the pasta two yolks and a whole egg, a cup of flour, cup of finely grained semolina, half a cup of unsalted butter and water are needed. Add a half-pound washed and coarsely shredded red radicchio, along with some salt and pepper according to taste. 2 tablespoons of minced parsley can also be used for garnishing. The prepared pasta has to be left for an hour wrapped up in a moist cloth and the lobster has to be boiled for removing the shell. Chopped lobster meat and grilled radicchio would then require a seasoning with olive oil, lemon juice, white wine, salt, and both the peppers. By melting the butter in a skillet and sauting the radicchio followed by a seasoning with the salt and pepper would complete the sauce. A Fly Fisher’s Respect for Nature

By Steve Robbins Are fly fishers truly good stewards of our God given natural resources? Id like to think so but Im not so sure that all of us are. I would venture to assume that in fly fishing, the gentlemans sport, we have the ethical fibers within us that drive us to take good care of our rivers, lakes and streams. Is this an accurate assumption? Over the last 10 years, fly fishing has certainly been one of the fastest growing outdoor activities in North America. With this growth were sure to attract those who are less ecologically conscious that wed like. We can strive to create public awareness in the fly fishing community yet probably the best place to start is with ourselves. Marjory Douglass wrote, “The future lies within the strength with which man can set his powers of creation against his impulses for destruction.” Now dont get me wrong. I would definitely never pass as a purist tree hugger. I do not intend to preach but I think there area few things that we can look at. It truly hurts me to be on a beautiful stream to come upon the discarded remains of human presence that include water bottles, beer cans and cigarette butts. I know youve all hear one of the backpackers code of ethics, “if you pack it in then pack it out”. One pressing issue to consider for us nymphers is lead shot. Is it an eco sin to use lead split shot instead of tin? Of course it is! Lead is toxic and kills living creatures and you cannot seriously consider yourself a conservationist if you are intentionally putting lead into nature. Now, the lead that is accidentally dropped into nature by fly fishermen is surely a fraction of one percent of the total amount of lead that flies out of shotguns nationwide and lands in the wild, so why bother us with the issue? Because every little bit hurts. How many times have you seen a wadded and tangled leader in the stream? Have you seen the horrible images of wildlife becoming tangled in the mass of monofilament? Zane Grey wrote, “If I fished only to capture fish, my fishing trips would have ended long ago.” And so it is with most considerate anglers. The thrill of the catch is often overshadowed by nature’s breathtaking grandeur. I love to catch fish as much as anyone. However, when Im all alone and think back to my fly fishing adventures, I think less of the individual fish that I caught than of the places that Ive been and settings in which Ive fished. Fly fisherman become a part of the waters they fish. As naturalist John Muir observed, “When one tugs on a single thing in nature, one finds it attached to the rest of the world.” We must approach our pristine waters with respect. If we do this, then perhaps we gain its total amount of enjoyment. The fish aren’t always biting, but Mother Nature is always watching. I tout use of each software to it’s fullest and to only purchase equipment that will actively be used. Is It Time To Revisit Your Marketing Strategy?

Some of you have spent this past year just trying to get bymaintaining the web site, sending out occasional press releases, attending various networking events. Yes, you’ve endured our slow-to-grow economy, but how far have you deviated from your marketing strategy in order to survive? And to correct your approach, what new marketing programs should you add and what should you do first? Simplify what seems like an overwhelming task by asking the following questions: 1. Has my competitive landscape changed over this past year? Like so many companies, you probably haven’t spent much time evaluating your competition over the past year, but instead relied on what could be outdated assumptions. Before you can identify the right marketing strategy, you must take another look at your competitors. Are their any new players on the block? Has anyone disappeared? It’s time to create a new competitor list, identifying the strengths and weaknesses of each one. How are they positioning themselves? Can you determine what makes them unique? Once you have these answers compiled, outline your strategy against each competitor. Write down your competitive rant (why a prospect should choose your company over the competitor). Circulate these findings to your team for their input and buy-off. At the end of this exercise you should understand the following: Where is my competition currently marketing? What are they doing well? Who are they targeting? How are they positioned? How do I position against them…or with them? 2. Is my target market still the same? During difficult economic times, many companies abandon their target market strategy completely. Watching the pipeline dry up prompts even the most steadfast to relinquish their strategic approach and start trying to sell to everyone, everywhere. Not only does this weaken the marketing programs you do implement, it makes you lose your focus and forget whom you should truly be targeting for long-term success. Before you determine what your marketing plan is, re-examine your target market. Supplement your existing customer data with a little market research. Check online at trade associations and market research firms for free information that provides insights into industry trends. Once you have re-established your market, create a detailed profile for each of the following: who purchases your product or service, who influences the purchase of your product or service, and who uses your product or service. Your profile should have enough detail around it that you could source a prospect list from it to support your marketing programs. At the end of this exercise you should understand the following: Who is my customer? Who are their influencers? Which of my products/services are they likely to buy? Where do they go to get their information? 3. Is my company and product/service positioning still relevant? You may have unknowingly diluted your company’s positioning over time. When leads are hard to come by, companies understandably (but unfortunately) try to be everything to everyone in order to win the business. It is time to restate what you bring to the table, that is truly unique, and how that uniqueness benefits your customer. Correct any confusion you may have created over who you are, what it is you do, who you do it for, and why people should buy from you. Create 25, 50, and 100-word statements that describe these elements in a succinct fashion. Once you have this determined for your company, create clear, concise descriptions for each of your products and services. Using this positioning consistently throughout the year will correct any misconceptions about what you offer and explain why your customers should buy from you. At the end of this exercise you should understand the following: Does my positioning statement clearly (Remember clear is always better than cute) communicate what makes my company different from the competition? Does it state how my offering can solve the pain they are experiencing? Once you have reassessed your competition, reevaluated your target market, and recreated your positioning language, you will have the strategic foundation you need to determine the marketing programs you need to implement in your marketing plan. This information will help you choose relevant and appropriate marketing vehicles, and then establish solid priorities. Think, Strategize, Act and COMMUNICATE.
